Job Description

The LCSW will provide a full range of social work services in collaboration with other CSMC staff to meet the psychosocial needs of the Heart Transplant patients/families/significant others/caregivers. Provides social work services and care coordination to
patients and their families with psychosocial needs. Active participation in Department's continuing education. Involved in
program planning. Will be integral part of transplant multidisciplinary team.


Qualifications

Education: 
Master's Degree - Graduate of an accredited School of Social Work with a master ' s degree License/Certifications: 
LCSW Preferred 
Experience: 
Two years in an acute care hospital setting with transplant experience preferred
Must possess a minimum computer competency comprised of a working knowledge of Windows or comparable system (specifically including keyboarding and mouse skills). As applicable to individual job duties and expectations, employee must additionally possess a working knowledge of word processing, spreadsheets, presentation development, e-mail, browsers, and online reporting.
