Job Description

A Pediatric Congenital Heart Nurse Navigator, guides patients and families through treatments and care experiences. You will play a pivotal role serving as direct liaison and point of contact for patients and their families, to provide resources and assistance in accessing clinical and supported care services offered in the Guerin Family Congenital Heart Program, Smidt Heart Institute. The position involves direct patient care, comprehensive coordination of care, and eliminating barriers to timely and seamless care.

Job Summary:

  • Establishes and revises the plan of care for appropriateness on an on-going and timely basis as directed by the patient’s care team. 
  • Coordinates patient care activities and management of clinical data throughout all phases of patient care from initial referral to follow-up. Arranges for clinical services, evaluates clinical data, and triages clinical questions/ concerns to the appropriate members of the interdisciplinary team.
  • Demonstrates clinical competence in the assessment and intervention for patient, family, or significant other requiring emotional support, coping support and care for significant life changes. Coordinates support services within the health system and community to meet patient/family needs.
  • Able to communicate effectively in English in person, writing, and on the telephone; work independently; perform basic math and statistical functions; multitask and think critically with ability to pay attention to detail and accuracy; work well under pressure; organize and prioritize workload.
  • Able to set goals and assess results in accordance with the highest standards.
  • Able to build consensus around a common vision.
  • The Nurse Navigator guides patients and families through treatments and care experiences.
  • Collaborates with clinical staff to provide care and referrals.
  • Leads process and quality improvement activities to provide the best possible care.
  • Able to prepare timely and accurate documentation and reports.
  • Deliver care with sensitivity and respect, advance care through development of new ideas and technology.
  • Helps coordinate and facilitate with patients’ part of research and clinical trials.
  • Consults with physicians and other health care professionals as indicated regarding patient care issues and quality of care issues related to patients. Supports the leadership and clinical teams by providing progress reports on patient care.
  • Actively participates in quality improvement measures and projects as requested and directed. Recommends, develops, and implements policy/protocol revisions and performance improvement activities based on current standards.

Establishes, coordinates and continuously evaluates supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, beliefs and values.


Qualifications

Educational Requirements:

Bachelor's Degree Required
Master's Degree Preferred

Licenses:

Current, unrestricted California RN license required
BLS from the American Heart Association or American Red Cross required Ambulatory Nurse Certification or other specialty nursing certification required (or willing to obtain within one year from start date)
PALS from the American Heart Association or American Red Cross required Ambulatory Nurse 

Experience:

Must have 3 years experience in an acute care setting (transplant, cancer, breast, emergency,
pre-natal); Preferred experience with program development and
implementation of navigator programs.
