Registered Nurse - Lung Transplant Coordinator

Summary of Essential Duties:

As a Clinical Program Coordinator - Transplant, you will be responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services—from initial referral to follow-up care in Lung Transplant. We are seeking an expert RN to take responsibility for the following.
  • You will serve as the Lung Transplant Coordinator for the interdisciplinary team, including: Physicians, Nurses, Social Workers, Dietitians, Pharmacists, Financial Analysts, Case Managers, and support staff.
  • Develop, coordinate, implement, and evaluate educational programs/projects for patients, personnel, and the community.
  • Collects/documents data to meet clinical, financial, and regulatory requirements.
  •  Participates in the development of guidelines, protocols, pathways, and other performance improvement activities.
  • Participates in research activities.
  • Serves as a patient advocate throughout the patient care process.
  • Represents the Medical Center at regional and national meetings/conferences.
  • The Transplant Coordinator provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan.
  • You will maintain competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds.
  • You will identify the ability of the patients to follow directions/instructions and give consent.
  • You will determine safety concerns and institute measures to provide a safe environment; and provide appropriate assessment, intervention and evaluation of patients with consideration of developmental achievements in the patient population.

Educational Requirements:

  • BSN degree required

License/Certification/Registration Requirements:

  • A current California RN License
  • Specialty Certification
  • BLS from the American Heart Association

Experience:

  • A minimum of 5 years of Nursing experience required
  • At least 3 years of Critical Care experience required
  • Organ Transplantation experience preferred
  • Proven knowledge of case management
  • Demonstration of excellent organizational skills as well as interpersonal skills
  • Full command (verbal and written) of the English language
  • Experience in data management

Physical Demands: (e.g., lifting, standing, walking)

  • Sitting, walking, standing, bending, reaching, and lifting up to 25 lbs. Exposure to normal office and clinic area noise.
