Do you work well in a large team environment patients where critical thinking is essential?

The role of the Program Assistant, Disease Management is to support the various disease management programs. This position will be responsible for helping to uphold our programs policies and procedures, support the day-to-day operations of our programs and may include special projects designed to improve the quality of service, care and efficiency delivered to Cedars-Sinai Medical Care Foundation patients.

Responsibilities include the following:
  • You will run various disease management registry reports and delegate/schedule
  • You will assist facilitate all classes
  • You will assist with scheduling, calling to confirm appointment day before class, arriving patients who have attended the class, arrange follow up appointments with referring MD. Send letters when appropriate.
  • You will coordinate and set up various classes
  • You will manage/schedule all referrals to the Diabetes Program; triage based on Diabetes Program Flow Chart
  • You will assist with referrals and referral reports for Registered Dieticians (RD)
  • You will Schedule appointments for PharmDs, RD’s and CDE’s
  • Liaison between care managers and clinical pharmacists
  • You will assist with checking Travel Benefits for the Travel Consultation and Immunization Program
  • Sort monthly DTM Referral Report by provider and distribute to appropriate PharmD and RD
  • Maintain and distribute updated DTM Agreement Signature Log
  • Obtain DTM Agreement signatures from physicians
  • Make DTM binders with protocols
  • Proficient with Microsoft Office (Word, PowerPoint, Excel)
  • Perform additional duties as assigned

Minimum job requirements:

  • High school diploma (minimum requirement) and college degree preferred;
  • Requires 1-2 years healthcare experience, preferably in a managed care environment; clinical experience as a medical assistant or nurse preferred;
  • Knowledge and understanding of medical terminology and spelling, ability to type at 60 wpm required;
  • Ability to interact effectively and appropriately via telephone, electronic communication, and in person with medical, clinical, and administrative staff;
  • Superior communication skills (verbal, interpersonal, written);
  • Ability to act professionally at all times;
  • Demonstrated commitment to superior customer service, including quality, care and concern with each internal and external customer;
  • Must be self-motivated and work with minimal supervision, yet able to recognize when it is appropriate to seek assistance;
  • Ability to operate fax machine, photocopier, printer;
  • Ability to keep accurate records and pay close attention to detail;
  • Ability to effectively manage and prioritize multiple tasks;
  • Proficient in Microsoft Word, Excel, PowerPoint, Access and the internet;
  • Detail oriented and well organized;
