We are seeking a compassionate, dedicated Nurse Practitioner to execute the following job duties with our outpatient liver disease population.
- Responsible for the planning and coordination of all aspects of the work up and care for the Hepatology patients throughout the continuum of services-from initial referral to follow up care.
- The NP is responsible for preparing the patient for outpatient clinic by reviewing the chart, performing history and physical examinations, ordering diagnostic tests, reviewing results, and reporting to the Physician as needed.
- Develops and implement patient management plans, records progress notes, and assists in provision of continuity of care.
- Instructs and counsels patients regarding compliance with prescribed therapeutic regimens.
- The Nurse Practitioner is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership and patient care management across the health care continuum from preventative and acute care through discharge in addition to outpatient management.
- The NP serves as a patient advocate throughout the care process, and represents the medical center at regional and national meetings/conferences.
- Master of Nursing Degree required
- Graduate of an accredited Nurse practitioner program required
- A minimum of 3 years of experience as a staff RN required
- A minimum of 2 years of acute hospital experience required
- GI experience strongly preferred
- Current State of California Registered Nurse and Nurse Practitioner license required
- National Certification or eligibility for certification as a Nurse Practitioner
- Current BLS, Current DEA (or eligibility to obtain) required
