Pharmacist Resident

The Pharmacist Resident, Ambulatory Care position and the related residency program is designed to provide the resident with one year of advanced ambulatory care practice experience. The program will provide training in ambulatory pharmaceutical care, primary care, and managed care. The resident will work in collaboration with other healthcare professionals (physicians, pharmacists, nurses, and medical assistants) to perform medication therapy management, patient education, disease-specific physical assessments, monitoring and follow up, provide treatment recommendations to other healthcare providers, and respond to drug information requests. The resident will also have the option to learn the administrative side of pharmacy in medical groups.


Upon receiving a patient specific written order for management under a specific protocol, the resident develops a treatment plan, which may include drug therapy and lifestyle modifications.

Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.

Identifies, resolves, and prevents potential and actual medication-related problems including untreated indications, improper drug selection, sub-therapeutic dosage, overdosage, failure to receive medication, adverse drug reactions, drug interactions, and medications without indication.

Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other drug-related laboratory testing.

Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADRs, eliminating unnecessary drug use and duplication.

Provides patient education related to the medications associated with the individual's disease state, medication side effects, and reasons for proper compliance.

Assures ongoing performance improvement of the pharmaceutical care services provided.
Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.

Active participation in EMR updates and new developments as pertaining to medications.

Performs other duties as required.

EDUCATIONAL REQUIREMENTS AND/OR PREFERENCES (Licensure, Registration, Certification, Experience, Other):
 Current California Intern license and Pharmacist license required midway through residency year.
 A Doctor of Pharmacy (Pharm.D.) degree preferred.
 American Heart Association (AHA) approved Basic Life Support (BLS) certification must be obtained within the probationary period.
 Comprehensive knowledge of medical terminology as related to drug usage.
 Comprehensive knowledge of common disease states and pathologies and ability to apply current pharmacological and biopharmaceutical principles.
 Ability to comprehend patient medical management plans.
 Ability to extract pertinent information from the patient or other sources and relate it to the medical management plan.
 Possess excellent verbal and written communication skills.
 Ability to serve as preceptor for teaching/training student programs.
 Knowledge of laws, regulations and standards relative to Institutional Pharmacy practice and prescription dispensing.
 Ability to prioritize workflow.
 Ability to recommend, develop and expand programs and services to meet the therapeutic needs of the patients.
 Ability to learn and adapt to the use of automated information systems in daily practice.
 Demonstrated competency in pharmaceutical calculations and pharmacokinetics.
