The Ambulatory Care Manager (ACM) Manager will assist in the daily operations of the ambulatory case management department. Primary responsibility is to provide oversight of the daily functions of the ACM’s. The ACM Manager conducts case management on patients in the Ambulatory care and Skilled Nursing Facility settings. CSMDN has developed a case management program that is geared towards the management of patients as outpatient. In collaboration with the CSMG, CSHA, Primary Care Physicians and specialists, this program is designed to assist patients at the time they are identified as a candidate for ACM program in assessing patient’s needs, coordinating care, communicating with health plans to determine benefits and referring to ancillary programs i.e. disease management, house call, and Supportive Care Medicine/Palliative Care to reduce over utilization and to decrease unnecessary emergency room visits and re-hospitalizations as well as ACE’s (Acute Care Episode/Inpatient Admission.

Essential job duties:

  • Complete assessment for current/past medical conditions with dates of diagnosis, psychosocial issues, medications, ADL’s, pain assessment etc
  • Monitoring of continued appropriateness of the services rendered i.e. home health, DME, outpatient therapy.
  • Act as a team lead to include overseeing the work of supervisor and the rest of the team, assigning, or allocating work to the team and ensuring tasks are completed according to deadlines and quality standards
  • Plans, leads, and directs work of staff to ensure goals and objectives are completed within established budget and deadlines are met.
  • Responsible for hiring, onboarding, managing schedules/attendance, personnel actions performance reviews, and performance improvement plans.
  • Supervises the day-today work of Supervisor, Care Manager’s, and Care Coordinator’s, assigns work, ensures tasks are completed and deadlines are met.
  • In addition, the ACM Manager is expected to be able to: 
  • Conduct documentation audits
  • Monitor ACM Management Dashboard in CS Link for new patient referrals, overdue goals, and overdue follow ups.
  • Maintain the ACM vision board/Daily Engagement System board updates
  • Present ACM workflows and processes to our Care Coordination Training for our new staff members for MDN
  • other duties as assigned  


Minimum job requirements/competencies:

  • Graduation from an accredited school of nursing
  • Valid California State RN license
  • 2-4 years of acute nursing experience
  • 2-4 years in a Supervisor or Manager role, and 4 years in Managed Care
  • Certified Case Manager (CCM) certification for RN’s (Preferred)
  • previous case management, utilization review, or discharge planning experience

Base Pay: $110,200.00 - $181,800.00
